Average Monthly Payment Program Application
The Average Monthly Payment (AMP) Budget Plan allows residential utility customers to pay a 12-month average payments to assist with personal budgeting. Customers must meet ALL the following requirements to participate in the plan:
Be a RESIDENTIAL utility customer of the City of St. George.
Setup Auto Pay through Paymentus (City's online utility payment service) with frequency "Bill amount on the due date".
Utility account must be in the customer’s name at the same address for at least 12 months.
The utility account must currently have a zero balance at time of sign-up.

I meet all the above requirements and wish to participate in the AMP Budget Plan.
I have, or will within the next 2 business days, set up AutoPay through Paymentus at https://ipn.paymentus.com/cp/cosg. I will schedule the payment frequency to be "Bill amount on the due date".
This is a
12-month average payment
program, and I am still responsible for the full amount of charges on my bill. This budget will take effect on the next bill if I have submitted this form at least one (1) week before my next billing date.
I understand the City’s utility billing system will look at my current charges plus the prior 11-month actual charges to calculate the average for that months’ payment, also factoring in approximately 1/12 th (0.08333) of the overage or shortage if applicable. This process is completed each month and the payment amount will generally change each month up or down per changes in my utility service used and changes in utility rates.
It is my responsibility to review my monthly bill for accuracy, actual current charges, and reserve (overages or shortage) balances. I will notify customer service if I have any questions about my bill.
It is my responsibility to ensure that my bank account or credit card is updated for any changes at least three (3) business days before my payment is scheduled. If two (2) payments are returned on this account, I understand that I will be removed from the AMP Budget Plan immediately and any shortage will become due. If removed, I acknowledge that I would not be able to sign up for AMP Budget Plan for 12 months, and that I would have to have good payment history at that time to re-qualify.
I request participation in the Average Monthly Payment Plan and give permission to the City Utilities Office to bill me accordingly. I agree to the terms and conditions of the Average Monthly Payment Plan. All the information provided by me in this application is true and correct and I understand that any false or misleading information shall be cause for the city to deny or cancel the Average Monthly Payment Plan and demand immediate payment of any amounts which are due.
